Professor Damian Griffin is a leading figure in orthopaedic surgery and a globally recognised authority in the field of hip arthroscopy and hip preservation. His distinguished academic and clinical career is rooted in an exceptional educational background and a commitment to advancing evidence-based surgical practice.
He began his academic journey at the University of Cambridge, earning a First Class degree in Physiology. He then went on to study Medicine at the University of Oxford, where he laid the foundation for his future surgical career. Professor Griffin completed his specialist training in orthopaedic surgery both in Oxford and the United States, gaining broad international clinical experience. His passion for musculoskeletal science led him back to Cambridge to complete postgraduate study and research in the clinical epidemiology of musculoskeletal disease.
In 2002, Professor Griffin left his consultant position in Oxford to become the founding Professor of Trauma and Orthopaedic Surgery at Warwick Medical School. Since then, he has established and led a thriving academic department with a focused research agenda on evaluating the clinical effectiveness of orthopaedic procedures. Under his leadership, the department has become a hub of innovative research, medical education, and clinical excellence.
As a practicing surgeon, Professor Griffin has dedicated much of his clinical work to treating young, active, and athletic patients with hip-related problems. Over the past two decades, he has developed particular expertise in hip arthroscopy and hip-preserving surgery. His clinical insights and academic leadership have significantly shaped the evolution of these subspecialties.
In 2016, he led an international effort to establish a global consensus on femoroacetabular impingement (FAI) syndrome, culminating in the publication of the influential Warwick Agreement in the British Journal of Sports Medicine. This landmark consensus document has since served as a global reference for clinicians treating FAI.
Further solidifying his role as a pioneer in the field, Professor Griffin was the chief investigator of the largest ever randomised controlled trial of hip arthroscopy. Commissioned by the UK’s National Institute for Health Research (NIHR), this landmark trial was the first to provide definitive evidence on the clinical and cost-effectiveness of hip arthroscopy in patients with FAI syndrome, offering critical guidance for the use of this increasingly common surgical intervention.
Professor Griffin continues to play a central role in global orthopaedic research and education. He is a sought-after speaker and educator, regularly lecturing at major international conferences and contributing to surgical training around the world. His clinical expertise draws patients from across the UK, Europe, and beyond, many seeking specialist opinions and advanced surgical care.
In recognition of his exceptional contributions, particularly through his work with the Army Medical Services, Professor Griffin was awarded the Order of the British Empire (OBE) in the Military Division of the 2022 Queen’s Birthday Honours List.

The Warwick Agreement
In 2016, Professor Griffin played a leading role in defining how femoro-acetabular impingement (FAI) can be recognised, treated and managed by the global medical community.
The FAI Consensus Meeting was held at the University of Warwick on 30th June 2016 following the Sports Hip Conference held at St George’s Park, Burton-on-Trent over 28th-29th June.
The aim of the meeting was to arrive at a consensus on the management of FAI in order to publish an article giving guidance as to what is a reasonable way to approach the care of an FAI patient. The FAI Consensus Meeting discussed the material presented during the 2016 Warwick Orthopaedics Sports Hip Conference together with the body of knowledge presented in a selection of academic papers and peer-reviewed articles, before the expert consideration of seven questions by the panel members.

UK FASHIoN RCT
Full randomised controlled trial of arthroscopic surgery for hip impingement compared with best conservative care
Professor Griffin led the largest ever randomised control trial of hip arthroscopy, and the first to demonstrate the benefits of the surgery for people with femoroacetabular impingement syndrome.
UK FASHIoN (Full trial of Arthroscopic Surgery for Hip Impingement compared with coNservative care) was commissioned by the UK National Institute of Health Research, the R&D arm of the National Health Service, with a budget of almost £2M. It was prompted by the need to provide an evidence base for the increasing use of hip arthroscopy in the UK (a need that is felt around the world), and was designed to compare the clinical and cost effectiveness of hip arthroscopy with ‘best conservative care’.
In UK FASHIoN we recruited 348 patients from 23 hospitals across the UK. The surgeons were all providing a hip arthroscopy service, and the physiotherapists delivering personalised hip therapy (PHT) were specially trained to do so. All of the patients had FAI syndrome, as defined by the Warwick Agreement, and all were randomised to either hip arthroscopy or PHT. This was a pragmatic trial, so we knew and accepted that some people might not get the treatment they were allocated to, that there might be delays in treatment, and that some patients might not follow all our advice. It is best to think of a pragmatic trial like this as being a test of the best strategy: When you decide upon which strategy to recommend to a patient in clinic, which will result in the best outcome, taking into account all of the problems and friction which occur in everyday clinical practice?

So, what did we find? In both groups, patients’ hip related quality of life increased, measured by iHOT-33 one year after randomisation. Mean iHOT-33 scores improved to 59 in the arthroscopy group and 50 in the PHT group. After adjustment for potential confounders, there was a greater improvement in those allocated to hip arthroscopy than PHT, with a mean difference of 7 points (p=0.009) in favour of surgery. This is statistically significant, but is also greater than the minimum clinically important difference of the iHOT-33, so patients will notice and care about it. This might not seem like a big difference, but remember that this is an average benefit of surgery, compared with a really good conservative care package. When you combine this with the real-world challenges of a pragmatic trial, which tend to dilute the effect, we can be very confident that surgery provides real benefit in most of our patients.
